2b09325fc1 Make markdown links consistent on cursor exits 
Make the versions of link with and without * behave alike:

-   don't set the 0 location so that the cursor exits.
    Having a 0 in the title is bad because titles are short.

-   either open a new line at end for both, or for neither

    The only case where the newline is certain is for link definitions.

fbbe71e534 Fix markdown fenced code blocks. 
Broken because backticks were interpreted as vim code execution.

Also simplified the trigger which was too long and unintuitive.

Rationale: it is more common to have links without title in Markdown.

b1d1961540 Remove self-closing meta tags on HTML5 templates 
In HTML5, it's not necessary, or common, to have self-closing tags. This
commit removes the self-closing meta tags for the html5 and html5l
Snipmate snippets.

ae0cca3345 Ultisnips: Common CSS snippets 
These already exist in the `snippets` style, but thse will not be loaded
by Ultisnips when working with SCSS files, due to how the extend feature
works.
